# Public Lead Forms

> Build hosted and embeddable lead capture forms in Leaf360 Mortgage CRM: templates, fields, approved domains, lead defaults, appearance and submission activity.

Public Lead Forms builds the forms that sit on your website and turn a visitor into a record in the CRM. Each form is published two ways at once: a hosted page you can link to, and a snippet of code you paste into a site you control. Submissions land as leads, or as business contacts, depending on how you set the form up.

Open it from **Settings**, under the **Leads** group, then **Open Public Lead Forms**. The browser tab reads **Public Lead Forms**.

  These forms are customer facing and they collect personal data: names, email addresses, phone numbers, income, employment and estimated credit range. Whatever notice, consent or disclosure your company is required to show alongside that collection is your responsibility to put on the page. The form builder doesn't add one, and it doesn't record a consent decision with the submission. Have your compliance team review a form before you publish it.

## The overview

The heading reads **Public Lead Forms**, with **Create hosted forms, control embed domains, and monitor submission activity** underneath, and a **New Form** button beside it.

Four numbers run across the top: **Forms** with **Hosted + embedded**, **Active** with **Live right now**, **Submissions** counting all time, and **Leads created** from those submissions.

### Your Forms

Below that, every form gets a row: its **Status** as **Active** or **Inactive**, its name with the form's web link underneath, its approved **Domains**, its **Submissions** count with how many leads that created, and its **Last activity**. Click the name to open the editor.

Seven icon buttons finish each row:

| **Edit form**            | Opens the editor                                                   |
| **Refresh token secret** | Issues a new submission secret for the form                        |
| **Delete form**          | Deletes it, after a confirmation that says it can't be undone      |

With none built, the panel reads **No public lead forms yet** and **Start with a hosted page, then copy the iframe snippet into a client website.**

## Starting from a template

**New Form** opens the editor on **Form style**, a gallery of five starting points. Pick one and it replaces the fields, steps, copy and defaults with that template's.

The two multi-step templates carry the mortgage detail: loan purpose, occupancy, property type, purchase price, estimated home value, loan amount, down payment, property state, estimated credit range, annual income, employment status, military or veteran status and timeline for the pre-qual; refinance goal, current loan balance, current rate, estimated home value, property state and credit range for the refinance.

## Building the form

### The copy

**Form name** and **Slug** sit at the top. The **Slug** is the last part of the form's web address — Leaf360 fills it in for you from the form name, unless you'd rather set it yourself. Below them are **Headline**, **Subheadline** and **Description**, then **Submit button text** and **Success message**, which is what a visitor sees after they submit.

**Form active** is a switch with its own line of explanation: turn it off to stop hosted and embedded submissions right away.

### The fields

Every field carries a **Label**, the wording the visitor reads, and a **Saved name**, the key it stores under. **Add field** appends a new one, **Remove** deletes it, and a form has to keep at least one.

Each field also has:

**Dropdown** and **Choice cards** add an options box underneath, one option per line.

<Note>
  **Hidden** fields aren't shown to the visitor but do submit with the form, which is how a landing page passes a campaign identifier through. Don't use a hidden field to collect anything the person hasn't been told about.

### Form behavior

The right-hand column starts with **Form behavior**, described as choosing what the form captures, who owns it, and how it displays.

* **Design** picks between **Conversational**, one big question at a time with icon choice cards, and **Classic card**, a compact card with a progress bar and stacked fields.
* **Captures** decides where submissions land: **Leads (borrowers)** or **Business contacts (partners)**.
* **Form owner** is either **Company form (no default assignment)** or **My form (submissions assign to me)**. The note underneath adds that forms created by loan officers are always owned by them, and their submissions auto-assign to them.
* **Display mode** is **Full page (hosted, shows logo)** or **In page (iframe embed, no logo)**.
* **Logo**, offered on full page forms only, is **No logo** or **Brand Wiki logo**. Where your Brand Wiki has a logo, it's previewed here; where it doesn't, the panel says so and offers a link to go add one.
* **Contact phone (shown top-right)**, again on full page forms only, appears in the form header as a tap-to-call link opposite your logo.

### Lead defaults

**Lead defaults** decides how a submission is labeled and assigned before anybody touches it.

| **Assign to**   | A searchable list of your team members, or unassigned                                   |
| **Tags**        | Comma separated, applied to every submission                                            |

### Embed security

**Approved website domains** takes a comma-separated list of the sites this form may be embedded on. **This is required before saving.** A form with the box empty gets refused with **Add at least one approved website domain before saving this form.**

Get this right before you publish. It's what stops your form from being lifted onto somebody else's site and used to post into your CRM.

### Appearance

**Primary color**, **Accent (light) color**, **Background color** and **Text color** each have a swatch and a text box, and **Border radius** takes a size, for example `8px`, controlling how rounded the corners are. A **Live preview** underneath shows the form as the hosted page will, with **Desktop** and **Mobile** buttons to check both, and step arrows on multi-step forms.

**Save Form** commits everything. Saving without a name gets refused with **Form name is required.**

## Where submissions land

A submission becomes a record in the CRM, decided by **Captures**:

* **Leads (borrowers)** creates or updates a lead, carrying the lead source, status, priority, assignee and tags you set as defaults. It shows up on [Sales Pipeline](/mortgage-crm/sales-leads) and [All People](/mortgage-crm/all-people) like any other lead.
* **Business contacts (partners)** creates a business contact instead, which shows up on [Contacts](/mortgage-crm/contacts).

Not every submission makes a new record. The activity view counts **Created** and **Updated** separately, and each submission is labeled with which of the two it did, so a form filled in twice by the same person doesn't leave you with two records to merge.

## Submission activity

The **Submission activity** button on a form's row opens a window described as reviewing recent public form traffic and whether each submission created a new lead or updated an existing one.

Four numbers head it: **Total**, **Accepted**, **Created** and **Updated**. Under them, each recent submission is listed with a status label, a result label, its timestamp, the name and contact detail that came through, the **Embed origin** it was posted from or **Hosted page**, any message the visitor left, and the error if one was rejected.

With nothing yet, it reads **No submissions yet.**

The embed origin is worth reading. A submission arriving from an origin you don't recognize means your form is embedded somewhere you didn't approve.

## Common problems

    Two things block it: **Form name is required.** and **Add at least one approved website domain before saving this form.** Both appear as messages in the corner of the screen.

  <Accordion title="The embedded form does not load on my site">
    Check the site's domain is listed under **Approved website domains**, exactly as it appears in the address bar, including any subdomain. Also check that **Form active** is on, since turning it off stops hosted and embedded submissions right away.
  </Accordion>

  <Accordion title="Submissions are arriving but nobody is picking them up">
    Set **Assign to** under **Lead defaults**, or set **Form owner** to **My form (submissions assign to me)**. Company forms with no assignee leave submissions unassigned in the pipeline.
  </Accordion>

  <Accordion title="The lead status I want is not in the list">
    That list comes from your company's own lead statuses, configured under **Lead Statuses** in Settings. Add it there and it appears here.
  </Accordion>

  <Accordion title="My Brand Wiki logo is not showing on the hosted form">
    A logo only appears on **Full page** forms with **Brand Wiki logo** selected. If your Brand Wiki has no logo, the editor says so and links you to where to add it. Embedded forms never show a logo.
  </Accordion>

  <Accordion title="A partner filled in my mortgage form and became a lead">
    **Captures** is set per form. Use the **Partner / Referral** template, or set **Captures** to **Business contacts (partners)**, for a form aimed at agents and referral sources.
  </Accordion>
